School Health Visit for Primary 1 to 6 Students

Dear Parent / Guardian,

1. Health Promotion Board (HPB) will be conducting annual school health visit at your child’s school from 8-Apr-2024 to 16-Apr-2024.

2. Parents of Primary 1 and 5 students, please access the Consent Portal to update your child's medical information and provide consent to receive the age-appropriate immunisations in the school. No further action is needed if this has been previously submitted and there are no updates.

3. During this visit, age-appropriate health screening and immunisation services will be provided for your child. Kindly refer to the attachment (Memo frrom HPB) for more details of the services.

4. If you do not wish for your child to undergo HPB’s health screening in school, please write to Contact_YPS@hpb.gov.sg
